A headlight switch manufacturer audit should determine whether the supplier can repeatedly build the approved design, detect process drift, control changes and trace affected product. A certificate, clean production area or successful sample is not enough. The audit must follow evidence from customer requirements through process planning, production, inspection, testing and release.

Define Scope, Criteria and Audit Method

State the site, processes, product family, shifts, outsourced operations and project stages included. Audit criteria may include contracts, drawings, customer requirements, approved samples, internal procedures and applicable quality-system requirements. Record exclusions and interfaces with sub-suppliers.

ISO 19011:2026 provides guidance for management-system auditing and includes evidence-based and risk-based approaches. It does not certify a headlight switch supplier or prescribe this scorecard. Use the edition and criteria approved by the purchasing organization.

Define sampling before arrival: which lots, dates, shifts, machines, operators, gauges and records will be followed. Select evidence by risk, recent change, complaint history and process variability rather than accepting only records chosen by the supplier. Use interviews, site observation, record trails and product checks, then distinguish objective evidence from opinion.

Scoring and Conclusion Rules

Score each question from 0 to 5: 0 no system or evidence; 1 largely absent or ineffective; 2 partially implemented with major gaps; 3 implemented with isolated weakness; 4 effective and consistently evidenced; 5 effective, sustained and improved using data. Calculate the weighted result as score divided by five multiplied by the assigned weight.

Set the purchasing organization’s pass score before the audit. A practical project rule may require both a weighted threshold and closure of all Major findings, but the buyer must approve the actual threshold. A Major nonconformance is a systemic absence, loss of control, product-safety risk, false record, failed traceability or condition likely to release nonconforming product. A Minor is an isolated lapse that does not indicate system breakdown. An observation is an improvement opportunity, not a substitute for classifying nonconformance.

Automatic rejection should apply only to pre-agreed conditions. Candidates include falsified evidence, refusal of required access, inability to identify affected lots, unauthorized product or process change, shipment of knowingly nonconforming product, or an uncontrolled safety-critical process. Document the rule, decision authority and appeal or re-audit path.

Decision Example

Assume the weighted score reaches the buyer’s threshold but one Major finding remains open. The supplier is not fully approved because the score cannot cancel the Major. An unvisited process or shift remains “not evaluated,” not conforming. Conditional approval must name the product, factory, process, quantity or period, open action and required closure evidence. Falsified records, failed traceability or unauthorized change still trigger the agreed automatic-rejection rule regardless of total score.

Follow the Quality-Planning Chain

The chain illustrates document linkage; actual controls and acceptance criteria must come from approved project documents.

Start with the process flow and physically follow one headlight switch from incoming material through molding, contact or mechanism assembly, electronics where applicable, final assembly, testing, labeling and shipment. The PFMEA should address credible failure modes for each operation. High-risk controls should appear in the control plan, be translated into usable work instructions and leave traceable inspection records.

Choose one characteristic such as terminal position, switch-state output, contact resistance or actuator function and trace it through all five documents. Conflicting symbols, frequencies, methods or reaction plans indicate a broken planning chain even when each document exists.

The wider automotive project trail should run from customer requirements -> APQP or project planning -> design and process risk -> PPAP or customer approval -> serial-production controls. The exact method is contractual. Where required by the customer or contract, verify the applicable APQP, PPAP, core-tool and customer-specific requirement records. Do not declare these systems mandatory for every headlight switch project, and do not treat sample acceptance as production approval unless the customer has defined it that way.

Review Measurement Systems and Process Evidence

Check whether gauges and testers have suitable range, resolution, accuracy and calibration traceability. For key dimensional or electrical characteristics, request the measurement-system analysis required by the customer, including GR&R where applicable. A calibration sticker alone does not prove that a method can distinguish acceptable process variation.

Review SPC and process-capability evidence only where the characteristic and data type support it. Cp or Cpk without stability, an appropriate specification and a capable measurement system can mislead. Confirm who reacts to trends, how product is contained and whether capability is recalculated after tooling, material or process changes.

Observe first-piece, tool-change, material-change and shutdown-restart verification. Compare the actual operator sequence with the work instruction. Sample records across shifts and include both accepted and rejected results. Audit sampling plans, escalation, stop rules and disposition authority rather than checking only completed forms.

Audit Product and Process Controls

Verify terminal and connector orientation, locking, retention and damage prevention. For each switch position, compare the approved function matrix with tester channels and actual results. Confirm that direct-load, relay-request, ground-side and BCM-coded designs are not tested with one generic circuit.

Review contact material, plating, spring, lubricant, resistor, LED or electronics and housing controls where applicable. Examine storage life, contamination prevention, electrostatic controls for sensitive electronics and lot segregation. Outsourced plating or component manufacture needs defined specifications, approval status, incoming evidence and change notification.

People, Training and Authorization

Sample the training matrix for operators, inspectors, maintenance staff and laboratory personnel. Authorization should match the task, equipment, product revision and recertification rule. Interview people at the workstation and ask them to explain defect examples, reaction plans and who may release held product.

Check how temporary staff, transferred operators and engineering trials are controlled. Training attendance is weak evidence if competence was never evaluated. Compare authorization status with actual production and inspection records.

Traceability, Recall and Change Control

Run a timed traceability exercise from finished label to component lots, machines or lines, test result, operator or authorization, rework and shipment. Then trace a selected component lot forward to all affected finished lots and customers. Quantities should reconcile, and unexplained gaps require classification.

A mock recall should test decision roles, communication, stock containment and retrieval of evidence, not only database search. Review changes to contact design, plating, terminal source, spring, lubricant, resistor network, electronics, software, housing resin, tooling, process, site or sub-supplier. Confirm risk review, customer notification when required, sample selection and reapproval before shipment.

Corrective Action and Closure

Every finding needs an evidence ID, requirement, objective evidence, risk, classification, containment, owner and due date. Root-cause analysis should explain why the system allowed the condition, not restate the defect. Permanent action must update the affected process and identify product or records requiring retrospective review.

Closure requires implementation evidence and effectiveness verification using a defined sample or time period. A revised procedure without trained users and compliant records is not closed. The final report should state weighted score, automatic-rejection status, open Majors and Minors, approved scope, conditions and re-audit requirement.
